Secondary Transition

Transition activities and services should begin no later than the first IEP to be in effect when the student turns sixteen, or younger if determined appropriate by the IEP team, and updated annually, thereafter.

Transition IEP Meeting Requirements

LPS contracts with the ESD 112 to offer Student Transition Educational Program Services (STEPS) - a community based program designed for 18-21-year-old students with adaptive delays from the districts of Southwest Washington.  STEPS offers the opportunity to participate in career exploration and receive specialized instruction, training and support including daily living, communication, self-advocacy and social skills.  If you have questions please contact your student’s case manager to inquire about the process.
